If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.
You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients should be aware of the following :-
We are operating an initial telephone triage assessment service. This means that when you contact the surgery for an appointment the reception staff may ask you relevant questions prior to appointing you to the doctor or nurse as appropriate. Please be aware all information is dealt with in the strictest confidence and can be very helpful when prioritising the appointments.
It should be noted that GPs are independent contractors and there may exist different prescribing policies between practices. As such we reserve the right to review medications you have been prescribed at any previous practice and amend your prescriptions according to our own policies. For instance, we do not prescribe regular benzodiazepines unless recommended by psychiatry. This may mean you medication list is altered on becoming a patient here.
Once you have handed your completed paperwork into reception we will endeavour to update your details on to our system as quickly as possible. This may take a week or two to complete and it may be a further few weeks for your previous records to be sent across from your previous doctors practice. This would not prevent you from accessing the services of the practice during this time but it may mean we will not have your full medical records to hand. Please also ensure you have an adequate supply of any repeat medications from your previous practice to allow this transfer to take place and if possible provide us with a copy of your repeat prescription list.
In order to register please come into reception and staff will provide you with registration forms and explain the registration procedure.
When you attend to hand in your registration form please bring proof of any medications you take regularly and also a form of photographic ID (passport/driving license) and a current utility bill.
